How health care law affects lives of 7 Americans

CHICAGO (AP) ? A father lost his job at a medical device company that is facing a new tax. A young woman got back on her parents' insurance and was able to get surgery for an injury that could have hobbled her. A part-time sales woman stopped putting off a colonoscopy and cancer screenings and saved nearly $3,000 because health plans now must pay for preventive care without co-pays. A business owner received a tax rebate for providing health coverage to her employees.

As the U.S. Supreme Court prepares to hear arguments on President Barack Obama's health care overhaul, The Associated Press spoke with a variety of people to hear their experiences so far with the landmark legislation, whose major provisions don't take effect until 2014. Reporters asked: How has the health care law affected your life?

Here are snapshots of seven Americans:

___

Name: Michael Esch

Home: Warwick, N.Y.

Age: 48

Occupation: Former middle manager for medical device company, now working as a hospital purchasing agent.

Insurance coverage: Paying out of his own pocket for COBRA insurance through his former company.

Esch, a father of three, lost his job in November in a layoff his employer said resulted from President Barack Obama's health care law. Medical device maker Stryker Corp. announced in November it intended to lay off 1,000 workers worldwide to save money ahead of a 2.3 percent tax on medical devices that starts in 2013.

The tax on medical devices is meant to help pay for expanding health coverage to uninsured Americans. The Obama administration argues device companies will gain in the long run as more patients become eligible to receive their products because they have insurance.

Esch was a middle manager who had worked for Stryker for six years. He helped develop a product known as the Triathlon Knee. Since the layoff, he's taken a salary cut to work as a hospital purchasing agent. He's still looking for a job with another medical device company.

He blames the medical device tax for the loss of his job, but he's grateful for the provision in the health care law that will allow his oldest child, now a college sophomore, to stay on his health insurance to age 26.

"We tend to forget that for every great idea there is a ripple effect through other sectors of a business," Esch said.

Economists say most companies should be able to pass on the bulk of the tax to customers, but the industry says it will squeeze profits and chill investment, hiring and innovation.

___

Name: Glenn Nishimura

Home: Little Rock, Ark.

Age: 62

Occupation: Consultant to nonprofit groups.

Insurance coverage: Uninsured since COBRA coverage from a previous job expired in May of 2009.

Nishimura has been uninsured for nearly three years. He lost his health coverage after he left a full-time position with benefits in 2007, thinking he could land another good job. The recession destroyed that plan.

He's been denied coverage because of high blood pressure and high blood-sugar levels. A provision in the national health care law gave his state $46 million to insure people like him who've been denied coverage because of pre-existing conditions.

But Nishimura said he can't afford the coverage. It would cost him about $6,300 a year in premiums with a $1,000 deductible, meaning he would pay the first $1,000 out of his own pocket before coverage kicks in.

He worries about suffering injuries in a car accident or falling ill before he's eligible for Medicare at age 65.

"I don't like feeling vulnerable like this," Nishimura said. "I'm completely vulnerable to some catastrophic problem."

Nationally, about 50,000 people with pre-existing conditions have signed up for the coverage available through the health care law, fewer than expected. The government has offered new options to encourage more to enroll. In another two years, he may be eligible for subsidies under the law for insurance.

___

Name: Samantha Ames

Home: Washington, D.C.

Age: 25

Occupation: Law student

Insurance coverage: Got back on parents' insurance, thanks to the health care law.

As a teenager, Ames was prone to ankle injuries playing catcher on baseball and softball teams. Last April, she tripped over her mini bulldog and badly injured her left ankle. Ultimately she needed surgery that cost her insurer $30,000.

But she considers herself lucky.

Only a few months before her accident, Ames had been able to get back on her parents' insurance, thanks to a provision of the health care law that lets young adults keep that coverage until they turn 26. Nationally an estimated 2.5 million young people have gotten insurance as a result.

Ames says it's unclear if the student health insurance she had been relying on previously would have covered her surgery. In any case, the copayments would have been steep. She would have had to postpone the operation, risking another ? potentially crippling ? injury.

"The fact that I was able to get on their plan is the reason I can walk today," said Ames. "Very rarely have I had something political affect me this personally."

___

Name: Sharon Whalen

Home: Springfield, Ill.

Age: 59

Occupation: Publisher of a weekly alternative newspaper

Insurance coverage: Small group plan.

As a co-owner of the Illinois Times, a weekly newspaper, Whalen wants to keep her small staff healthy. So she and her business partner provide them with health insurance and pay half the cost of premiums for their 10 employees.

Keeping that employee benefit is getting more and more expensive. The company saw a spike in premium costs after one employee's child had chronic health problems.

With costs climbing, the company switched to a managed care plan with higher copays for some services in 2009. Whalen's company also contributes less than it once did to cover the premiums of employees' family members.

The health care law brought some relief: a tax credit for small businesses that provide health coverage. The Illinois Times qualified and received a $2,700 tax credit last year.

"We see ourselves putting that money right back into the company," Whalen said.

Whalen heard about the tax credit from a health care advocacy group, not from her accountant.

"I had to practically beg them to look at this," Whalen said. "They weren't familiar with it."

The Obama administration has proposed expanding the number of businesses eligible for the credit, and simplifying the paperwork.

___

Name: Melissa Pearson

Home: Prineville, Ore.

Age: 53

Occupation: Retail sales, part time.

Insurance coverage: High-deductible plan purchased on individual market.

A few years ago, Pearson's doctor ordered her to have a routine colonoscopy. It's one of several colon cancer screening methods highly recommended for adults ages 50 to 75.

Pearson kept putting it off, in part because of the cost. Her high-deductible health insurance plan requires her to pay the first $5,600 out of her pocket each year. She knew the colonoscopy would be expensive and figured she'd be paying.

Then she learned that a provision in the health care law requires health plans to cover all costs for preventive care including colon cancer screening ? with no out-of-pocket costs to the patient.

"That's what made me make the appointment," she said. She also scheduled a mammogram and cervical cancer screening, which also are covered preventive services under the law. In all, she saved nearly $3,000 in out-of-pocket costs last year because of the Affordable Care Act.

"I said to my sister, 'Thank you Obamacare," Pearson said.

The Obama administration says the Affordable Care Act provided about 54 million Americans with at least one new free preventive service last year through their private health insurance plans.

But Pearson is worried that covering preventive services will mean her insurance premiums and her taxes will go up. "It's being paid for by somebody," she reasoned. She recently talked with a student from Norway who told her about the tax levels in that country. "I'm fearful our world will turn into that."

___

Name: David Zoltan

Home: Chicago

Age: 34

Occupation: Field marketing coordinator for a public relations firm.

Insurance coverage: Federally funded health plan for people with pre-existing conditions.

Zoltan lost a job and his health insurance during the recession. His diabetes sent him to the emergency room three times when he ran out of insulin during the two years he was uninsured.

In 2010, he was one of the first to sign up in Illinois for a new health insurance program for people with pre-existing conditions. The Affordable Care Act set aside $196 million for the state of Illinois to start the program.

Zoltan now pays about $1,848 a year for that coverage. The plan has a $2,000 deductible, meaning Zoltan also pays that amount out of pocket before the coverage starts.

Zoltan has found work, but his new job doesn't include health benefits, so he'll stay on the federally funded health plan.

"As a diabetic, I never again want to be without health insurance," Zoltan said. "Anything is better than not having coverage at all."

He is watching the Supreme Court as it considers the law. The requirement that Americans buy health insurance is under constitutional scrutiny. Zoltan believes the individual mandate is needed to spread the risk among the well and the sick, and keep insurance affordable.

___

Name: Carol McKenna

Home: Pembroke Pines, Fla.

Age: 70

Occupation: Retired.

Insurance coverage: Medicare Advantage plan.

McKenna and her husband Morty have noticed that Medicare's "doughnut hole" is shrinking. The coverage gap in Medicare's prescription drug program ? dubbed the "doughnut hole" ? caught Morty in December last year. But once there, he received a 50 percent discount on brand-name drugs and other discounts on generic drugs thanks to Obama's health care law.

Last year, he received a $250 rebate check provided by the new law for people in the doughnut hole. Under the health care law, the gap will be gradually phased out by 2020.

Warnings about possible cuts to Medicare Advantage plans caused by the health care law haven't come true, Carol McKenna said. Their health plan still includes extra benefits such as fitness center membership.

She said she's grown weary of the political debate over the health care law.

"I've been following it somewhat. Then it got so convoluted and out of control during the elections that I stopped paying attention," McKenna said. "I don't want to hear it anymore. All they're doing is sniping at each other."

Analysis: Nimble neighbors outshine Brazil on growth

MONTEVIDEO (Reuters) - Swift growth by nimble Latin American countries like Chile, Colombia and Peru has put renewed focus on regional heavyweight Brazil, whose relatively closed, high-tax economy is now sputtering below its potential.

The three Andean countries grew more than twice as fast as Brazil last year and are expected to outpace it again this year. Brazil has also lagged its peers in the BRIC club of emerging market heavyweights that includes China, India and Russia.

Officials in the Andean countries say they have benefited from low public debt loads, fiscal surpluses that allow them to invest heavily, and an aggressive pursuit of free-trade deals with big countries that have made their economies among the most open in the world.

In Brazil, politics have at times stymied ambitious fiscal reforms to eliminate the deficit in a country with powerful public sector unions.

Strong business groups anxious to protect their lucrative positions in the domestic market of 200 million people have resisted free-trade pacts. That has left consumers in the lurch: the iPhone 4S was retailing for $1,250 at an outlet in Brasilia in February. They sell for half as much on Amazon.com.

"There are two visions," about which economic model to follow: an open one or a closed one, Colombian Finance Minister Juan Carlos Echeverry said at the weekend at a meeting of the Inter-American Development Bank in Uruguay's capital Montevideo.

"The message we send to our producers is you are in the world, not just Colombia. The future is about being competitive and this is as painful as giving birth."

The three Andean countries grew between 6 and 7 percent last year, well above Brazil's 2.7 percent.

Brazil's economy has expanded steadily over the last decade and it grew 7.5 percent in 2010, leading some economists to say it had finally buried its history of slow growth. But with last year's sharp decline, those worries are back.

Most economists now say Brazil will grow only around 3.3 percent this year. Peru expects to grow up to 6 percent and Colombia is so confident of its expansion that the central bank has been boldly raising interest rates.

"In open economies like Peru there are fewer market distortions, unlike economies which tend to close themselves and create new artificial barriers," Peruvian Finance Minister Luis Miguel Castilla said in Montevideo. "That also means the capacity of companies in open economies to adapt in a context of international competition is much greater."

Castilla said the risk for small countries that rely on commodities exports is that they can suffer shocks when prices fall, but that they also can recover quickly.

Echeverry and Castilla declined to explicitly pinpoint Brazil's challenges but touted the success of their efforts to streamline taxes, lower tariffs and ramp up public investments.

Brazil lacks a major free-trade agreement despite exports that have surged over the last decade. Its Byzantine tax code soaks up about 34 percent of gross domestic product and requires companies to hire armies of accountants.

Its debt load of 37 percent of GDP, though it has fallen from more than 50 percent over the last decade, is costly to finance because of interest rates that are among the highest in the world and a net public sector deficit that historically has crowded out investment by the private sector.

Years of shortfalls have left infrastructure bottlenecks that contribute to inflation and which the government is racing to eliminate before Brazil hosts the World Cup in 2014 and the Olympics two years later.

Brazil's companies have been mostly sheltered from competition for years. Now, what the government has dubbed a global "currency war" has left its currency, the real, near all-time highs and undermined the competitiveness of Brazilian industry.

Economists worry that 75 percent of Brazil's exports are tied to commodities prices.

"This leaves us vulnerable. We need to diversify our exports and we must make our industries more competitive," said Elcio Gomes Rocha, chief economist at state-owned Banco do Brasil.

He said capital investments make up only 19 percent of Brazil's GDP and that the minimum the economy needs is about 24 percent. Brazil also has a relatively low domestic savings rate.

"You grow when you save," Colombia's Echeverry said. "The Asian countries keep on growing because they have savings rates of 30 or 40 percent and investment rates of 40 to 45 percent. It's not like this if you just consume and don't save."

Mauro Leos, regional credit officer for Latin America at the ratings agency Moody's, said it is a medium-term worry for Brazil.

"Brazil is a large economy, it's a BRIC. However it's one of the few countries in the sovereign universe where you have investment rates below 20 percent. Those countries that have low investment rates tend to be poor, small or not rich. It's something that's strange."

'REASONABLE GROWTH'

Though Brazil has made enormous progress over the last decade - lowering the poverty rate and joblessness to record lows - its reforms never went as deep as those in Chile and Peru, where right-wing authoritarian governments in the 1980s and 1990s slashed tariffs and overhauled public pension liabilities.

Pension and social security liabilities have long been a major cause of Brazil's continuing public sector deficit, which governments since the return to democracy in 1985 have narrowed to 2.4 percent of GDP by making piecemeal changes.

But talk of sweeping fiscal reforms has run into political resistance. The ruling Workers' Party has close ties to public sector unions, which were instrumental in helping it win the presidency in 2002.

Opposition to shrinking the state became a rallying cry during President Luiz Inacio Lula da Silva's re-election campaign in 2006, when the party seized on a plan by the economics advisor to opposition challenger Geraldo Alckmin to drastically cut spending 10 percent across the board.

Lula's aides said the Alckmin plan would throw the economy into a tailspin.

But Miriam Belchior, Brazil's planning minister under President Dilma Rousseff, says that is changing and that fiscal and monetary policy now work together to help reduce the lofty benchmark interest rate, which has contributed to an overvaluation of the real and undermined the competitiveness of Brazilian exports. The Selic benchmark rate is currently at 9.75 percent after a series of cuts over the past six months.

"Right now it's possible to have this new mix between fiscal and monetary policy," she said. "This is the big change."

Rousseff froze 55 billion reais ($32 billion) in spending this year, mainly on administrative and discretionary costs, while preserving outlays for public investments. That followed a 50 billion reais reduction last year.

Belchior said the government has also sent a bill to Congress that over the long-term would help control pension costs for public sector workers, a major drag on fiscal accounts. It has cleared the lower house but not the Senate and relations between the government and Congress are strained.

Brazil's economy grew at an annual average of 3.8 percent over the last decade, but Peru had an average rate of 6.4 percent.

Brazil's former central bank chief, Henrique Meirelles, tried to downplay worries about modest growth. "Brazil still has a reasonable trend growth rate," he said. "The country has the conditions to grow but has challenges."

He said Brazil's economy was not driven only by exports. "It's very important that the domestic market is kept strong and that has been one of the reasons for strong growth."

Rousseff's government was criticized as protectionist by policymakers at the IADB meeting for pressuring Mexico last week to restrict its auto exports to Brazil.

Chile and Peru - and to a lesser extent Mexico and Colombia - have arguably the world's most ambitious free-trade agendas.

Their free-trade pacts stretch from China and Japan and South Korea, to the United States and Europe. More pacts with smaller trading partners have been signed or are in progress with countries like India.

EU trade ministers agreed on Friday to approve a free trade pact with Colombia and Peru.

"Once the EU accord, is ratified 90 percent of our trade will be covered by some kind of preferential accord," Peru's Castilla said. "I think this an advantage that our country has relative to our neighbors who don't."

5 Things You Must Know Before Tapping With Children and Teens ...

Tapping with kids can be a very rewarding experience. In many cases kids take to tapping much faster than adults because they are more willing to try new things and aren?t hung up on why something is working. They just want to feel better.

Here are five things to keep in mind when teaching kids to tap. (Note: All five lessons below are hard won. I know them to be true because I have done the opposite of all 5 at one point.)

1) They Are Smarter Than You Think

I recently spoke to 600 middle school students about bullying. To start the presentation I talked about why bullies bully. I talked about what happens on an emotional level and how that affects our choices. At the end of the presentation the kids broke up into to small groups with their adults to talk about what they can do to stop bullying in their schools. After the small group time a number of the adults walked up to me to express how surprised they were with the complexity of thoughts the kids had, the emotions they experienced, and how overall thoughtful they were.

Kids don?t have the vocabulary to express their emotions and thoughts the way adults do, but they are thinking real thoughts about their lives, their emotions, and who they are. Give them space to talk about their emotions, teach them how to talk about them, and they will surprise you (in really good ways).

2) You Must Be Authentic Or They Will Tune You Out

Kids today are very savvy because of their access to information and technology. They are treated by big business as a market place and they are constantly sold to through TV and on-line. Because of this they have very sophisticated BS detectors and they can tell when someone is not being authentic with them.

It is very tempting to want to be cool and hip when talking with kids (especially teens) thinking you are ?speaking their language?. It is just the opposite. They don?t want some line or for you to talk like them. They want someone who is just being who they are (even if that means being your dorky self).

You can?t ask a kid to honest with their emotions and be comfortable inside their own skin unless you are willing to try to do the same.

3) Your Job Is To Love Them; Not Be Loved By Them

When we are working with teens or children we are doing it because we want better for them. That is the goal. Nothing more. Nothing less. We are not trying to be their friend and we are not trying to gain their admiration. We have our own friends and peers for that. This doesn?t mean that we act like jerks or that we don?t care, forgetting to treat them with respect. Sometimes wanting the best for someone is asking them to do hard things and things they might not enjoy.

To help them heal we need their respect, not their love and affection. It is okey to be comfortable with that notion.

4) Teach Them Tools

Kids (and most people) don?t care how or why tapping works. They are just looking for something that makes them feel better. When working with kids and teens give them tools. They don?t need theory. They want to be better, happier, and healthier. Once they know the tool works they might ask you how it works and why.

I love coming up with little processes that are easy to follow.

For example, a great way for parents to teach kids to tap for themselves is have them tap and explain four things whilst they are tapping.

  • * What was the best part of the day?
  • * What was the worst part of the day?
  • * What are they looking forward to tomorrow?
  • * What are they worried about tomorrow?

Very simple. It is easy to remember and easy to do. Once they have tool they can take care of themselves.

5) Start With How The Emotions Feel In The Body

As I said above, kids (and many adults) don?t have the skill or vocabulary to talk about their emotions as emotions. I like to have them talk about how the emotions feel in the body to help them to tune in.

They might not have the vocabulary to talk about the disappointment that comes with poor grades vs. fighting with a friend. They might call both of these emotions ?mad?. But if you get them to talk about how it feels in the body they are going to be able to be more specific. By describing where the emotion is in the body (head, hands, stomach?) and how it feels in these places (heavy, tight, hot?) they are going to be much more successful with their tapping.

Go For It

You don?t have to be perfect when working with teens or children. They are looking for adults who care about them and care about their well being. Show up from a place of love, treat them with respect, and be ready for lots of questions and you will be a great tapping teacher and tapping role model for them.

What have found works well when tapping with kids? What mistakes have you made that you have learned from? Click here to add your own thoughts and comments or read what others have to say. I would really love to hear what you think!
